HOW DO I REGISTER FOR AN OHIO MEDICAL MARIJUANA CARD?
STEP ONE:
Make an Appointment with a Certified to Recommend Physician
Visit a Certified To Recommend Physician who can confirm that you have a qualifying condition for medical marijuana. The physician will create your profile in the Patient & Caregiver Registry.
Your CTR Physician will need one of the following forms of ID: the patient’s valid Ohio driver’s license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport.
Your CTR Physician may also ask for medical records pertaining to the patient’s qualifying condition. You can often find a summary of relevant information through your MyChart account with your healthcare provider.
Patients registered with a special status of Veteran or Indigent are eligible for discounts at dispensaries and reduced state registration fees. Patients wishing to apply for Veteran or Indigent status will need to bring additional paperwork to the physician appointment and inform the CTR of their status. Additional details can be found here.
STEP TWO:
Confirm and complete your registration for the program through the Patient & Caregiver Registry.
Your CTR Physician will enter your information into the patient or caregiver registry and you will receive an email with instructions. The link found in the activation email will be active for 90 days. However, the link can only be accessed once and you will be asked to create a password for your Registry profile. Once logged in you will have to complete your patient or caregiver application and pay the annual registration fee ($50 for patients and $25 for caregivers).
While the link in your activation email will no longer be active, you can log back into the Registry by visiting www.ohiomedicalmarijuanaregistry.com and entering your username (email address) and password.
**Note: Patients who qualify special Veteran or Indigent status may have their state registration fee reduced by 50%. Patients seeking qualification as Veteran or Indigent Status should not submit payment until documentation of their special status has been received and approved by the Board of Pharmacy. The “Activate Card” button is not available while the status is pending.
**Note for caregivers: While a patient’s card is active immediately upon submitting payment, a caregiver’s card may take up to longer for activation.
Once you have received your Patient or Caregiver Registry Card, you will be able to purchase at The Landing Dispensary or any other dispensary licensed by the State of Ohio Board of Pharmacy.
STEP 3: Simplify Your First Visit
Complete New Patient Intake Form Digitally
Follow the link to complete New Patient Intake Form on your phone or computer. Fill out your New Patient Intake Form from your phone or computer! We can send a digital New Patient Intake Form using intakeQ that you can complete electronically, sign, and return before your first dispensary visit!
